U.S. Senator Jerry Moran, the ranking member of the Senate Veterans’ Affairs Committee, has been informed by the Department of Veterans Affairs (VA) about an issue with an electronic fax system. This problem has led to 875 files going missing after being sent to the VA Eastern Kansas Health Care System from community care providers.

The lost files date back to June 11, 2024, and include essential medical documentation, care referrals, authorizations for care, and other necessary information for coordinating veteran health care.

Senator Moran stated, "I have asked VA to quickly review every file and take all appropriate actions to mitigate patient harm that has or may result from this system failure." He also urged a review of the other 27 health care systems across the Veterans Health Administration using the same electronic fax system. "My office will remain in contact with VA until the full scope of this problem is known and all necessary corrective actions are taken," added Sen. Moran.

Veterans concerned about potential delays in their care due to this issue are advised to contact their VA care team. For additional assistance, they can reach out to Senator Moran’s office.
